Femur interlocking done one week back. Post op noticed the cervical fracture. He also developed fat embolism for which he is treated .should I operate or conserve it.

  Gaurav Jain WITH A SUCH A LARGE BONE GAP AND ALSO THE FRACTURE NECK , ITS NOT GONNA UNITE AT BOTH SITES ... BEST IS TO OPERATE ... I BELIEVE IF THE PT IS TOO OLD (TO HAVE SUCH FRACTURE IS MORE COMMON IN ELDERLY PT ) AND PT BELONGS TO HIGH SOCIO- ECONOMICAL STATUS , GO FOR THE LONG STEM ARTHROPLASTY AFTER IMPLANT REMOVAL ... IN CASE OF A POOR PT , I AGREE WITH THE ABOVE TECHNIQUES .... DEFINITELY HIP SPICA IN PRESENT DAY ROLE IS NOT AN OPTION .... MEANWHILE I RECOMMEND YOU TO START HER/ HIM ON ANTI OSTOPOROTIC TREATMENT .... I BELIEVE IT TO BE SOLE REASON FOR HAVING SUCH FRACTURE .... WISH YOU LUCK FOR YOUR DECISION

  Lokesh Bhatia Well, I did I series Double Neck shaft Fractures, Presented in Regional conference, Incidence of missing Fracture Neck Femur is to the tune of 20% worldwide to prevent such missed Neck Femur, One centre recommends and they are practicing doing CT of Pelvic region routinely in all fracture shaft femur,to rule out Neck fractures.

  Lokesh Bhatia Iatrogenic Fractures while doing ILN is also not uncommon, one of the series it is reported to have happened in 15% cases, Well how to prevent this from happening in our hands, By adequate Raeming, may be up to 1.5 to 2 mm more then the nail diameter being used,Nail Should pass comfortably and not require hammering, If you were required to hammer then its very high chances of iatrogenic.

  Lokesh Bhatia Now What to do or what I have observed other Surgeons doing in such cases try passing miss a Nail Technique, what Dr Prawin Kanabar has advised, But eventually if you are not able to do for any reason, be Prepared to replace the Nail with Reconstruction Nail, Address the Neck shaft Fracture Perfectly.
