Ahsan Choudhary i think he may need long time for union. i use external support (back slab) to avoid loosening if i apply ex fix for same type of fracture

Rajeev Dwivedi it's latest and extended application of locking system as ex-fix .indication for such use is judged on individual basis .but its not entirely wrong.

Pradip Nemade Internal fixator used as external fixator !!!
Conceptually right!

Advantages:
1. Less weight
2. Multiple angle screws put from a small area( Not possible for traditional ex fix
3. Connecting system is close to skin..may add to biomechanical stability if properly applied

Disadvantages:
1. The threaded portion of the screw is projecting outside skin.... so the contaminant can trickle down along the threds {Partialy threaded screws (may be those used for far cortical locking) can be utilised to avoid this issue}
2. Technically little difficult to apply
3. not versatile like traditional ex fix
4. can not be extended if needed
5. Has to be applied very close to skin..so sometimes wound management may become difficult
6. Cost???

Future:
1. Partially threaded screws with long thread length
2. Comparative bio-mechanical studies

Kir Pat This is bilateral inj and coexistenet or severity of softtissue inj and looking at status of contra limb and may be compartmental softtissue primarily ,this is justifiable..I agree with Pradip Nemade, but add other parameter of exfix well maintained, 1)bone pin interface2)bone bar distance3)screw diameter.4)rod or here plate breadth5)no of pins..rafting screw esp in subchondral yield more sterngth than one pin in exfix..6)pin pin distance...7)on tension side..It will give good result..in union.

Sanjay Tripathi My Dear friends...we must keep our eyes nd ear open to welcome new nd innovative ideas...in era of information boom we should not be much traditional...if any proceedure helping our pts though not much supported by books..we should use it ofcourse after satisfying our own conscious..

Rajiv Maharjan The big disadvantage is you can not dynamise the frame unlike AO frame. This is nothing just to drain the patient money...If he can use the AO fixator in other limb, why not in this lime?

Rajiv Maharjan Locking plates are not designed for that purpose. It's like taking a medicine for not getting its therapeutic effects rather to get its side effects/adverse effects.

Rajiv Maharjan ye saab paisa ke khel he, implant walo se commission/cuts ke liye log taraha taraha ki nautanki karte he. Even in so called developed countries many surgeries are implant industry driven. THIS IS A BITTER FACT. WHY EVERYBODY IS PROMOTING SUCH THINGS ??????

Imran Malik Concept is according to AO manual in LCP with LISS plate done in MIPO.

But technically seeing that pic and knowing that the cortex and plate distance is too much in this case giving a weak construct. Leading to non Union.

Also no X-rays have been put up so can't comment on it 100%.

Also in this idea you can not dynamize the construct, where as most forms of ex fix you can.

I would be delighted to know the outcome of this case with pre op and post op X-rays with follow up X-rays too.
Skin condition too.

Mayank Bansal I use this type of construct in many patients where I can not use an internal fixation..

Threaded screw may lead to infection, But otherwise better control of fragments,
Its not against the principal of Locking plates..

Those who r bothered bout money, Let me tell them, Here In my place, I use, Used plates.. So cost of screws only ..
